Capture forms and surveys
Build forms and surveys, publish them as a link or an embed on your own website, and see every response land in Prevarro.
This article is being updated.
Capture is where you build the forms and surveys people fill in to reach your business. You publish one as a page of its own or embed it on your website, and each response lands in Prevarro as a contact you can follow up.
By default, the Owner and Managers can create, build, publish and delete forms. Everyone else can look, and sees those buttons turned off.
Your forms and surveys
Go to Capture to see every form and survey, with its type, status, number of slides and number of submissions. Filter by Type (Form or Survey) or Status (Draft, Published or Archived), or search by name.
Select a row to open it. Its menu also offers Open, Duplicate, Publish or Unpublish, and Delete. Deleting a form deletes its responses with it and can't be undone, so Prevarro asks you to type delete to confirm.
Start a new one
- Go to Capture and select New capture surface.
- Pick a starting point: Lead enquiry form, Multi-slide survey, Detailed brief or Start from scratch. Everything stays editable.
The builder opens on the new form.
Build it
A form opens on three tabs: Build, Responses and Analytics. Changes save as you go. The header shows Saving... while a change saves and Saved for a moment once it has, or Not saved if it didn't save.
On the Build tab:
- Slides. A form is a row of slides across the top. Select Slide to add one. To move a slide, drag it along the row. Or move to it with the Tab key, press Space to lift it, use the arrow keys to move it, and press Space again to drop it (Escape cancels). The contact slide always stays last. Turn it on or off with the Contact slide on / Contact slide off button. A form keeps at least one slide.
- Fields. Add questions from the list on the left. Use Search fields to find a type. Select a field on the slide to edit it on the Field tab.
- Where answers go. Under Maps to, choose where each answer is stored, such as a contact's email. Use Search the data registry to find a field. An email field goes to the contact's email and a phone field to the contact's phone on their own. Every other field starts on Survey response, which keeps the answer with the response. If you choose Not mapped, the answer is still shown on the response, marked Not mapped, but isn't stored anywhere else. If a destination can't take that kind of answer, the form won't save until you change it.
- Slide settings. On the Slide tab, add a Privacy message to the contact slide. Select Survey logic to show a slide only when an earlier answer matches, or to send people to a later slide or a web address. Rules run top to bottom, and the first match wins. Select Save logic to keep your changes.
- Opportunities. On the Slide tab, under Opportunity on submit, pick a Pipeline and Stage. Each submission then opens an opportunity there, or joins the contact's open one in that pipeline. Leave Pipeline on No opportunity to add the contact alone.
- Design. On the Design tab, set the accent colour, font, corners, card style, logo and submit button.
Switch to Preview to try the form. Choose Classic or One question, and New visitor or Known contact.
Publish it
- Select Publish in the header. It waits until your latest changes are saved.
- Under Hosted page link, set the end of the form's own web address. Select Copy to copy the link.
- Under Embed on your site, select Copy to copy the code for your website. The embed can't be broken by your site's styles.
- Select Publish now. You'll see Published - version with its number.
Visitors see the version you last published. Changes you make afterwards reach them when you publish again. Each response records the version it was answered on.
To take a form down, choose Unpublish from the More actions menu (⋯) or from its row. Archive makes a form read-only. Choose Restore to edit or publish it again.
What your visitor sees
The form saves answers on the visitor's device as they type, and submits even if they go offline. After they submit, they're told where it stands:
- Thanks - we've received your submission.
- Received - we've saved this safely and it will be processed shortly.
- Saved on this device - we'll send it as soon as you're back online, with a Retry button.
If the form can't load, they see This form cannot be loaded right now, and anything they typed stays on their device until it goes through.
Responses
The Responses tab lists every submission, newest first. Narrow it by date (All time, Today, Last 7 days or Last 30 days) or by source (Hosted, Embed or API).
A complete submission creates or updates a contact, matched so the same person isn't added twice. A visitor who leaves partway shows in the list as Didn't finish - no contact yet, and as Partial when you open it.
Select a submission to see every answer beside where it was stored, and a Captured automatically section with the page it came from and where it was placed. To remove it, choose Delete from its menu. This can't be undone.
The Delivery column shows whether each submission has landed. If it reads anything other than Landed, see Why hasn't this submission landed?
Analytics
The Analytics tab shows how many people viewed the form, how long they took, how many completed it, which slides they reached, and where completed responses came from. Choose a Served version to look at one version, or All versions.
Sending submissions from another system
If another system, such as your own website code, sends submissions to your business, it needs a key. People who manage Capture's settings set this up:
- Go to Settings → Integrations.
- Under Developer & data, open the Ingest API card.
- Select New API key and give it a Key name.
- Under Capture surface, choose the form it sends to. A key sends to that one form alone. Generate stays off until you've named the key and picked a form.
- Select Generate. Copy the key straight away. It won't be shown again.
A system that signs what it sends uses a signed source instead. Under Signed sources, select New signed source, name it and pick its form in the same way. Its secret is also shown once. Choose Rotate if it's lost.
The card reads Active while it has a key or source that hasn't been revoked, and Not configured otherwise. Held submissions lists anything still waiting to land. For what to do there, see Why hasn't this submission landed?
